GDPR information obligation on the conditions for the processing of personal data:

1. The Controller of your personal data is Akademia WSB (WSB University) with its registered office in Dąbrowa Górnicza 41-300, ul. Cieplaka 1c. To contact us, send an e-mail to: rektorat@wsb.edu.pl or call +48 32 262-28-05.

2. Please be informed about the appointment of the Data Protection Officer, the function of which is performed by Martyna Kucharska-Staszel. You can contact her by sending an e-mail to: iod@wsb.edu.pl or by calling +48 513-457- 575.

3. Personal data will be processed:

a) on the basis of art. 6 sec. 1 (a) GDPR, that is, participation in conference streamed online - XVII European Kaizen Congress.

b) on the basis of art. 6 sec. 1 (a) GDPR, in connection with art. 81 of Act of 4 February 1994 on Copyright and Related Rights, that is, for free and time-unlimited use and dissemination of my image by WSB University with its registered office in Dąbrowa Górnicza, ul. Cieplaka 1c, in the form of films, digital photography in any format and in the following media: the website of WSB University, the social media of WSB University due to the promotional and information activities.

c) on the basis of art. 6 sec. 1 (a) GDPR, that is, in order to present educational offer related to the activity of WSB University, in particular information about organized scientific events and/or training by text messages, e-mails and traditional mail;

d) on the basis of art. 6 sec. 1 (a) GDPR, that is, in order to send additional marketing and commercial information as well as information about events organized by WSB University and institutions cooperating with the University by text messages, e-mail and post.

4. Recipients of your personal data will be the following entities:

a) a platform service provider for the online broadcast of the event: ClickMeeting/Zoom, YouTube.

b) University employees or persons employed by the Controller on the basis of civil law contracts - on the basis of authorizations granted by the Controller to process personal data pursuant to art. 29 GDPR.

c) entities cooperating with the WSB University and processing personal data on the basis of a previously concluded contract for entrusting the processing of personal data in accordance with art. 28 of the GDPR and entities operating on the basis and for the purposes specified in accordance with generally applicable provisions of law;

5. Personal data will be stored for the purposes of data processing on the basis of granted consent, which will be processed until the consent is withdrawn, without affecting the lawfulness of the processing on the basis of consent before its withdrawal or until the purpose of processing ceases to exist.

6. Providing personal data indicated in point 3 (a) is voluntary but necessary to participate in the conference streamed online, entitled XVII European Kaizen Congress.

7. Subject to point 7, you can also participate in the event via the YouTube channel, then your personal data will not be published, and in the case of participation on the ClickMeeting or ZOOM platform, in the absence of consent referred to in point 3 (a). b please do not turn on the camera during the event.Providing personal data specified in point 3 (c), (d) is voluntary, however, lack of consent or its withdrawal will prevent the implementation of the purpose on this basis.

8. Lack of consent or its withdrawal for purposes indicated in point 3 (b), (c),(d) has no effect on personal data processing for the purpose indicated in point 3 (a).

9. Personal data will not be processed in an automated manner and will not be the basis for automated decision making, including profiling.

10. The controller may transfer your personal data to a third country within the use of the ClickMeeting/Teams platform, the service operator transfers the data only to providers that guarantee a high level of data protection on the basis of previously concluded agreements on the entrustment of the personal data processing and in the use of the open YouTube channel due to the transnational nature of data flow within YouTube using contractual clauses used by YouTube approved by the European Commission and decisions of the European Commission stating an adequate level of data protection in relation to specific countries in accordance with the rules set out by YouTube at https://www.youtube.com/intl/pl/about/ or wer/2801895?hl=pl or https://www.privacyshield.gov/eu-us-framework and in the scope using the "ZOOM" service will transfer personal data to a third country, because the service is provided by the US supplier.

11. In connection with the processing of personal data by the Controller, you are entitled to:

a) request access your data, GDPR art. 15,

b) request the rectification your personal data, GDPR art. 16,

c) request the deletion of your personal data - GDPR art. 17,

d) request the restriction of the processing of your personal data, GDPR art. 18,

e) object to the processing of personal data – GDPR art. 21

f) the right to withdraw consent to the processing of personal data at any time.The withdrawal of consent to the processing of personal data does not affect the lawfulness of the current processing of such data,

g) lodge a complaint with the supervisory authority for the compliance with personal data protection regulations, i.e. the President of the Personal Data Protection Office, ul. Stawki 2, 00-193 Warszawa, kancelaria@uodo.gov.pl (pursuant to art. 77 GDPR).
